If you are new to working with me, please be sure to read the following forms, and bring them to your first session. This will help ease you in and allow as much time as possible to focus on you.
The following resources are there to best inform you as a client on what to expect from me as your psychologist.
If you feel that you would like to collaborate your session with another provider, simply fill out the following form.
About these forms
These forms explain how your information is collected, stored and kept confidential, and what you can expect from your psychologist. Completing them before your session means more of your appointment time can be spent on what matters to you.
The Client Consent Form and Client Contact and Information Sheet should be completed by all new clients. The Limits of Confidentiality and the APS Charter for Clients explain your rights and the circumstances in which information may need to be shared. The Authorisation for Release of Information is only needed if you would like Franco to share information with another professional, such as your GP.
